If you think your child may benefit from Special Education services and you live within the Fallbrook Union Elementary School District boundaries, you will need to first register your child for school here. You will need a copy of the child’s birth certificate, immunization record, and proof of residency within the FUESD boundaries.

Your second step will be to write a letter addressed to the Director of Special Education detailing your concerns (not those of a third party) and the type of assessment you are requesting. Include the following details:

• Child’s legal name
• Child’s date of birth
• Grade
• Parent Name
• Home address
• Telephone
• Signature and Date

The letter is to be presented to the school site office or to the Special Education Office at 321 Iowa Street, Room 401, Fallbrook, CA 92028.
An evaluation plan will be completed within 15 days of receipt of the request. The evaluation will be completed within 60 days of receiving the signed consent form from the parent.

Special Education Referrals for Students Not Currently Attending a Public School

Children with suspected disabilities may be referred for Special Education assessment even if they are not currently enrolled in a public school. If you are concerned that your child may have a disability that could negatively impact their educational progress, you are encouraged to contact the appropriate school as outlined below.

Getting Started

To initiate the assessment process, please be prepared to provide basic demographic information typical for school enrollment. Even if you do not intend to enroll your child in FUESD, this information is necessary to ensure we have the required details to begin the evaluation process.

Determining Which School Site to Contact

• You live within FUESD boundaries AND your child attends a private school within FUESD boundaries: Contact your FUESD neighborhood school of residence. If you are unsure which elementary or middle school serves your neighborhood, use the School Locator Tool on the Fallbrook Union Elementary School District website.

• Your child attends a private school within FUESD boundaries, BUT you live outside FUESD boundaries:  Contact the FUESD school that serves the geographic area where the private school is located. Entering the private school’s street address into the district’s School Locator Tool will help you identify the correct FUESD school site to contact.

• You live within FUESD boundaries, BUT your child attends a private school outside FUESD boundaries:  Contact the public school district in which the private school is physically located.
